Hello, my name is Jacquelynn, and I will be your Registered Massage Therapist today.
The treatment will involve a combination of relaxation and therapeutic techniques. While generally safe, these techniques may occasionally cause mild discomfort, pain, light-headedness, or dizziness. I will regularly check in with you throughout the session to ensure your comfort.
You may undress to your comfort level for the treatment. If you would like lotion applied, I recommend undressing from the waist up or to your underwear. If you prefer, I can work over your clothing as well. You will be fully draped with a blanket and sheet at all times, with only the area being treated exposed.
If you decide not to proceed with treatment, please be aware that your current discomfort or symptoms may persist. I can, however, refer you to other practitioners or provide you with homecare exercises if desired.
Please note that if you wish to include treatment of sensitive areas such as the chest wall, gluteal region, or inner thighs, additional written consent is required. You have the right to pause or stop the treatment at any time.
By signing below, you agree and consent to the assessment and treatment, including, if necessary, treatment of sensitive areas mentioned above. You acknowledge that your personal health information will be collected and kept confidential, except where disclosure is required by law. You also consent to your medical information being shared with other healthcare providers involved in your care.
Treatments may be eligible for coverage under extended health care plans. It is your responsibility to verify your individual coverage details with your provider.
As you prepare for your treatment on the table, I will step out briefly to wash my hands and will announce when I return.

Clinic Policies & Professional Boundaries Agreement
This clinic is a professional, therapeutic healthcare environment. My goal is to provide safe, respectful, and ethical care for all clients. Romantic and sexual advances are not permitted.
All client interactions must remain appropriate to a therapeutic context. Friendly, neutral conversation is welcome, as is discussion related to treatment, health, wellness, or general topics.
Personal questions about my private life, relationships, are not appropriate.

Failure to comply with these boundaries and conduct expectations may result in early termination of the treatment session, full payment due for the scheduled appointment, termination of the therapeutic relationship, and refusal of future bookings.
Maintaining clear boundaries is part of my responsibility as a regulated healthcare provider and is essential for ethical, safe, and effective care.

Cancellation Policy
Appointments are reserved exclusively for you. Due to limited availability and the difficulty of filling appointments on short notice, a minimum of 24 hours’ notice is required for all cancellations or rescheduling.
Cancellations made with less than 24 hours’ notice, late arrivals that significantly shorten the appointment, or missed appointments (no-shows) will be charged 100% of the scheduled service fee.
This policy allows me to manage a sustainable schedule and continue providing high-quality care, particularly as my availability will be more limited following medical leave.
